aggressive

According to this theory, even distant metastases are the result of direct extensions of local involvement (affecting the breast, the chest wall, axillary and supraclavicular lymph nodes, or any combination of the sites). The Halstedian approach thus dictated that aggressive local therapy for control of disease in the breast, chest wall, and regional lymph nodes would have a substantial effect on survival. His ideas also provided justification for ever more radical breast-cancer surgery.

I'm almost sure that here aggressive means "more severe, intensive, or comprehensive than usual in extent"

But since many of my classmates (in fact, all of the other four) oppose to my translation; They insist that aggressive here related to "a medical procedure which penetrates or breaks the skin or a body cavity". They said it's approximately surgical. So I'd like to hear your opinions about it.

Which interpretation do you like?

Hi Maple, In this context, "aggressive" implies risk of certain degree. To me, it infers negative connotation, and has no relevence with being "comprehensive". In finacial terms, if I say I have an "aggressive inverstment porfolio", I mean exposure to certain volatility of monetary gain and loss which simply means "risk".
