Generally speaking, the sentence structure should be "If blah blah blah, then blah blah blah."

Also, look at your "it" and "they". People are not "its". That is, people are not "things" or "its".

So, we get the following:

However, if they don't have appropriate plans for health insurance, then they won't be able to afford the medical treat when they are old.

Or, if you are speaking about a company and its employees.

However, if the company doesn't have appropriate plans for health insurance, then its employees won't be able to afford the medical treat when they are old.
