"is fully well-formed". Do we need hyphen here is it an adjective, Please advise.

Yes, the hyphen is needed, so that we can understand the subordination of 'well' to 'fully'.

Logically, however, it leaves much to be desired. Why not 'perfectly-formed'? What else could 'fully well' mean?
