Addressing a doctor?

Hello guys:)
Where should the commas be in the sentences below?

Doctor, "name," is good in his job.
Dr. "Name," is good in his job.
We all love Doctor "name."
We all love, Dr. "Name."

By writing the title without the abbreviation, should we or should we not insert a comma before the title?

This subject has been confusing me for a long time, so please excuse my stupidness Emotion: big smile
