Please, how to read the following addresses (and floor) ?

1) 350 5th Avenue, NyY 10118 = ?;

2) 37th Street = Thirty-seventh street (?);

3) 106th floor = ?

And how to read this:

- N/R train to Madison Station or 4/5 train to Green Station =

Finally what's the meaning of: "West Side Service"? E.g.: "Bus M 6 (West Side Service)."
