Could you tell me if this is correct? Mainly concerning the plural form of "change". The intent was to say that many changes are made to many addresses.

ok, here goes:
1. Changes have been made to the list of addresses.
2. Please see below for address changes. OR Please see below for changes made to the list of addresses.
3. Please let me know if there are any addresses changes OR Please let me know if there is any address change?
4. changes of address / changes of addresses / change of addresses

Which is more appropriate? Are all of them appropriate?
