Adding past/present participle at the end of a sentence

I once was passing by a hospital specialized in back/disc surgery and was shocked to see a back patient.

I am trying to complete the said sentence by adding more details, at the end, about two separate actions the back patient was engaging in at the moment of my passing. The following are the two actions of the patient.

1.hunched over with one hand on his ailing back for support
2.puffing on a cigarette

Not knowing the correctness of it, I've just simply added the two different information in the order I listed above, using the conjunction "and". And I'd like to know how well the whole sentence scans with the English experts presiding the forum.

I once was passing by a hospital specialized in back/disc surgery and was shocked to see a back patient hunched over with one hand on his ailing back for support and puffing on a cigarette.
