I don't see auxiliary be (is, are, was, were, etc.) lurking about anywhere.

Must be active.

"An incident occured". What type of voice is that?

Occur is an intransitive verb.


Only transitive verbs can have the passive voice form.
Intransitive verbs cannot because they do not take an object.
