A pen has been given to Ram by me.

The active object (a pen) becomes the subject.

The active verb (have given) becomes the passive verb by adding the auxiliary "been" (has been given)
(Note that the active subject was the first person I, so the verb took the first person with "have"; the passive subject is now a third person it, so the verb becomes third person with "has.")
