Active Voice

Hi,

I knew passive voice form is 'SUB+WAS/WERE+P.P'

but in a video i watched on youtube someone says "The mattresses in the hotel room was crawling with bugs "

The above sentence is not in Passive form. it should be "The mattresses in the hotel room was crawled by bugs "

Is sentence said in the video wrong grammatically?
