active versus passvie

Some verbs like 'rip' mean 'to tear or to be torn ' ... so what's difference between active voice and passive voice in meaning?? for example:
'Her clothes had ripped'
'Her clothes had been ripped '

ripped and  been ripped have exatcly the same meaning (to be torn) ..so what's difference???
