ACKNOWLIEDGEMENT DISBURSEMENT ACCOUT

I hereby authorize Mr. Lee Kok Heng to act on my behalf of arrange and remit the amount of RM 6000 to my undersign domestic maid's recruiter named Siti being the fee and cost of disbursement to the domestic maid to be placed under my employment.
Upon the completion of employement contract 2 years, undersign domestic maid will bear the partial cost that undersign domestic maid had agreed to bear from any parties including the agency.
I further agreed not to hold any responsible on the authorized person for any matters arising out of this authorization letter.
