Acheivements and duties

If I want to list bullet points of an organization's daily/monthly tasks, should i start the bullet point with "carry out such and such" or" carries out such and such", and if i want to list the achievements (which are done already) should i use the past tense of the verb? like (did, prepared, accomplished...) or keep it in present simple form?

thank you
