Abbreviations, periods and consistency

I've noticed that the use of periods in abbreviations is often inconsistent.

For example:

Los Angeles = L.A. (more common periods)
New York City = NYC (usually no periods)
United States = U.S. (I see "US" just as often, but I've been told U.S. is correct)
Federal Bureau of Investigation = FBI (periods less often)
Any Police Department = PD (no periods)

How does one determine when to use periods in an abbreviation?

I just checked on Wikipedia - it seems there is indeed a lack of consensus on this matter: http://en.wikipedia.org/wiki/Abbreviation#Periods_.28full_stops.29_and_spaces
