Obesity is caused by a combination of (an-the) excessive food intake(s) , poor eating habits and (a-the) lack of physical activity.
If I use intakes(if it is right to use), then I don't need either of them (a-the)as in habits, THEN what do I do for lack of.... ?

The first and third ones are clear and understandable to me, but for the second , why do you use an article for one item only?

There is no need for consistency. We make a (usually subconscious) decision on whether or not to use an article each time we want to use a noun.
