A sorry excuse for a human

Hi! So, the thing is that I came across this phrase one day and started thinking what it actually means and how to use it. I did some googling and got confused.

I have three questions. What does "sorry excuse for a something" really mean? Can you use whatever adjective and noun you like? (terrible excuse, sad excuse, boring excuse / a girl, a teacher, a parent..) On a similar note, is it possible to use an expression "a sloppy excuse for a surgeon"?

While many combinations are potentially possible, I think we consider "sorry excuse for a ..." as a fixed expression.
