A request for sponsorship

Recently I had a close friend pass away from a 7 year battle with melanoma cancer. We are having a bowling tournament/fundraiser for his family to help with the cost of bills and to help with their 4 young kids. I would like to approach several sunblock companies and ask for sponsorship and possible sunblock donations. What kind of letter would I write for that? Do I need to have any specific forms?
