In a laboratory, I want to state that, after making a test, the head of the team should produce a report about it.

What is the correct sentence to say so (if any ...)?

1. “ ... the team leader shall issue a report by each test conducted”

2. “ ... the team leader shall issue a report for each test conducted”

3. “ ... the team leader shall issue a report per test conducted”
