A question about two sentences

Could somebody tell me what the differences between the two sentences below?

Sentence 1: No one has really understood me.

Is " I've ever been to the USA twice." a correct sentence?

Or I could only put it like " I've been to the USA twice."?

Or I could only put it like " I've been to the USA twice."? This is fine.

You could also say 'I've only been to the USA twice'.
