A little

About "I'm a little tired", does it only literally mean that I'm a little tired?

Or can it be a modest way of saying "I'm very tired"?

g. prone to understatement, or not wanting to be seen to make a fuss, might use it that way I guess. This is more to do with people not saying exactly what they mean for whatever reason than with any particular linguistic property of "a little".
