But it sounds like the list would just say eg

4 times.

Compare "Here is a list of when they have voted on an application".

This list would say eg

Uncountable nouns in the singular come after "amount of".
amount of sugar, amount of wine, amount of time

Countable nouns in the plural come after "number of".
number of streets, number of bread sticks, number of times

That said, "how many times" is fine for your sentence; "amount of times" is wrong; "number of times" is right.

I think you meant to say "a list of voters and how many times each has voted on an application".
