a formal email

hi dudes
im writing a very formal email to a professor,asking him to provide me with the questionnaires he mentioned in his book while he left their places blank saying he will put them in the book in the next edition. i am using this sentence: Also, we request you to inform us of any other questionnaires in conjunction with autism that we might find helpful for this research, like those you left their places blank in your book to fill in later. now, do you think this is proper to say it in this way? i need your urgent help

well, you may be right about the first thing, but is it necessary to take it too much tough and formal while writing in a forum?(i am sure you got much more experience than me)
