a cousin

Hi.

He is a first cousin of James.

If James has only one cousin, can I say "a first cousin" or should it be "the first cousin"?

Thank you very much.

Yes, but one has to realize that in the western culture, first cousin doesn't only mean # 1 in arrival, but how closely you are related to the person. The children of your parents
