'First date heartache'. Should it actually be hyphenated? 'First-date heartache'?

Yes.

Here's my take. first date heartache refers to the first heartache I've had on/before/after a date. first-date heartache refers to a common anguish on/before/after a date. We do have a 'bedtime' story that is one word. I've never heard of a 'bathtime' story (and our spell-check just told me it doesn't work - so a hyphen might be better).
