A(After had) is necessary in the following sentence?If answer is yes,why?Because  we have 2 substances in this sentence.
Bilirubin and albumin levels had a substantial rise in samples containing heparin while urea level was decreased significantly compare to serum.

Yes. "rise" is a singular countable noun and therefore needs an article or other determiner. It has nothing to do with the fact that there are two substances. However, "had a substantial rise in samples ..." seems slightly awkwardly phrased. Does it just mean "rose substantially in samples ..."?

There are various other problems. "urea level was decreased" should be "urea levels decreased"

Do you mean generally, or in this particular sentence?

Generally because The passive voice emphasizes the product of the action.For instance,the results have been analyzed or the urea level was decreased.On the contrary,the active voice emphasizes the performer of the action which is not important
