A question about the word "or"

My daughter had the following math problem:


Write the answer as a fraction or as a mixed fraction in simplest form.

1. .25

Her answer was 2/8.

My question is about the math instructions. Does the phrase “in simplest form” apply to the word fraction before the word “or”? I am a programmer and in my world the phrase “in simplest form” would only refer to the part of the sentence after the word “or”. Therefore the answer 2/8 is correct because it is a fraction and does not have to be in simplest form because it is not a mixed fraction.

I'm not sure what difference they make between a fraction and a mixed fraction but if I were to answer it, my answer would be "1/4" as that is the simplest fraction form for 0.25.
Maybe they are trying to say that there are two ways of saying the same thing. Just like there are the 'Continuous' Tense and the 'Progressive' Tense which are the same thing but have different ways of saying it.
